Please ensure to click the link in your reminder email or text and complete the form as soon as possible. Everyone needs to complete these forms for every appointment, even if you have been vaccinated against Covid-19. Our reception staff are extremely busy and contacting patients that haven’t completed their online forms take a lot if time that could be spent on other tasks.
